FAQs

Do I need to see a doctor before starting physical therapy treatment?

Answer: Currently in Illinois you may be evaluated by a physical therapist without a prescription for physical therapy, but treatment can only begin with a prescription from your doctor.

What should I bring?

Answer: You’ll need to bring your doctor’s prescription for physical therapy, comfortable clothes for exercising, insurance card, ID card, new patient intake form (download and fill out) and MRI/X-Ray/Surgical written reports if applicable.

What can I expect on my first visit?

Answer: The therapist will perform an initial evaluation of your condition assessing range of motion, mobility, stability, strength, flexibility and functional movement. A treatment plan will be developed to work towards meeting your goals while respecting healing time and realistic outcomes. For postoperative patients we stay in close communication with your surgeon to meet expectations and ensure proper progression towards quality outcomes.

Do you take insurance?

Answer: We are currently in-network with most BCBS plans and Medicare. We do not partner with HMO plans. We also accept self-payment for out-of-network plans.

Where are you located and how can I contact you?

Answer: Our physical therapy facility is located at 293 Northfield Road, Northfield, Illinois 60093. To schedule an appointment or for more information please call (847) 386-6310 or send us a message here.
